Objective: To determine the views of people with multiple sclerosis (MS) and professionals in relation to confidentiality, consent and access to data within a proposed MS register in the UK. Design: Qualitative study using focus groups (10) and interviews (13). Setting: England and Northern Ireland. Participants: 68 people with MS, neurologists, MS nurses, health services management professionals, researchers, representatives from pharmaceutical companies and social care professionals. Results: People with MS expressed open and altruistic views towards the use of their personal (...) information to facilitate service provision and research, placing trust in responsible guardianship and legitimate use of their information. Participant’s proposed that people with MS should be able to select their individual level of involvement in a register using levels of consent. It was agreed that access to the register should be governed by a guardianship committee composed of a range of stakeholders. People with MS did not wish their details to be used by marketing agencies and did not consider this a legitimate use of their data. Whilst participants were positive of the role a register could play in promoting research, participants felt that access to data by pharmaceutical industries should be administered by the guardianship committee. People with MS are concerned should their employers be able to access their personal information. Professionals were more cautious than people with MS in their approach to the use of patient personal data within a register. Conclusions: Whilst all stakeholders were positive of the benefits of an MS register, development of such a resource must incorporate robust data security and guardianship measures in order to ensure that, whilst opportunities are maximised, risks to the privacy of individuals and legal challenges to professionals are avoided. Understanding terrestrial carbon metabolism is critical because terrestrial ecosystems play a major role in the global carbon cycle. Furthermore, humans have severely disrupted the carbon cycle in ways that will alter the climate system and directly affect terrestrial metabolism. Changes in terrestrial metabolism may well be as important an indicator of global change as the changing temperature signal. Improving our understanding of the carbon cycle at various spatial and temporal scales will require the integration of multiple, complementary and independent methods (...) that are used by different research communities. Tools such as air sampling networks, inverse numerical methods, and satellite data allow us to study the strength and location of the global- and continental-scale carbon sources and sinks. Bottom-up studies provide estimates of carbon fluxes at finer spatial scales and examine the mechanisms that control fluxes at the ecosystem, landscape, and regional scales. Bottom-up approaches include comparative and process studies that provide the necessary mechanistic information to develop and validate terrestrial biospheric models. An iteration and reiteration of top-down and bottom-up approaches will be necessary to help constrain measurements at various scales. We propose a major international effort to coordinate and lead research programs of global scope of the carbon cycle. (shrink)

Green, but it is made clear that he had many colleagues. For the Idealists the state was not merely an instrument, it was also an end. It provided a focus for a sense of community. The religious doctrines and metaphysical theories of the Idealists led them to emphasize the state.

These two fragments, the earliest surviving examples of Hegel’s political theory, have not before been available in English. Though each is obscure, the germs of many of Hegel’s ideas are visible, including “spirit.” Annotations and appendices round off each fragment. An excellent index allows for cross-referencing. Moreover, H. S. Harris provides introductions to each fragment. That for the “System of Ethical Life” exceeds 90 pages. All told, more than two-thirds of the book is devoted to that fragment, but Harris is (...) right to say that is it illuminated by the “First Philosophy of Spirit” and vice versa. (shrink)

This memoiristic essay is a contribution to the Common Knowledge symposium titled “Fuzzy Studies: On the consequence of blur.” While probing his personal memories and making a case for devaluing our intellectual constructs, the author, an anthropologist, examines paintings by Paul Cézanne and Pieter Bruegel, poems by Wallace Stevens and W. H. Auden. The essay argues that each self-deluding “reality” we construct is only temporary, destined to fall back into the elusive, undifferentiated zone of overlap and ambiguity from which it (...) has emerged. Therefore, the author urges, we should temper the intellectual “rage for order” with an openness to chaos and contingency, along with sustained and careful attention to creative works and religious practices in which the mind appears to grasp its limits. In this way, we may substitute what Stevens called “ghostlier demarcations” for our clear, exact, and self-deceptive certainties. (shrink)

Fulford (bio)AbstractA recent study of the relationship between spiritual experience and psychopathology (reported in detail elsewhere) suggested that psychotic phenomena could occur in the context of spiritual experiences rather than mental illness. In the present paper, this finding is illustrated with three detailed case histories. Its implications are then explored for psychopathology, for psychiatric classification, and for our understanding (...) of the concept of mental illness. It is argued that pathological and spiritual psychotic phenomena cannot be distinguished by 1) form and content alone (as in traditional psychopathology), 2) by their relationship either with other symptoms or with pathological causes (as in psychiatric classification), or 3) by reference to the descriptive criteria of mental illness implied by the “medical” model. The distinction is shown to depend, rather, on the way in which psychotic phenomena themselves are embedded in the values and beliefs of the person concerned. [End Page 41] William James (1902), for example, argued that “in delusional insanity, paranoia as they sometimes call it, we may have a kind of diabolical mysticism, a sort of religious mysticism turned upside down” (426). Other commentators have noted a wide variety of phenomena, such as time distortion, synesthesias, loss of self-object boundaries and the transition from a state of conflict and anxiety to one of sudden “understanding,” all of which are reported in both spiritual and psychotic experiences (Buckley 1981; Watson 1982; Wapnick 1969; and Wootton and Allen 1983).Yet the similarities notwithstanding, the distinction between these two kinds of experience can be crucially important. Spiritual experiences, whether welcome or unwelcome, and whether or not they are psychotic in form, have nothing (directly) to do with medicine (Fulford 1996a). It would be quite wrong, then, to “treat” spiritual psychotic experiences with neuroleptic drugs, just as it is quite wrong to “treat” political dissidents as though they were ill (Fulford, Smirnoff and Snow, 1993). Pathological psychotic experiences, on the other hand, or psychotic symptoms, are by definition a proper object of medical treatment, sometimes even against the wishes of the person concerned. This book offers a close-up look at theological education in the U.S. today. The authors' goal is to understand the way in which institutional culture affects the outcome of the educational process. To that end, they undertake ethnographic studies of two seminaries-one evangelical and one mainline Protestant. These studies, written in a lively journalistic style, make up the first part of the book and offer fascinating portraits of two very different intellectual, religious, and social worlds. The authors go on to (...) analyze these disparate environments, and suggest how in each case corporate culture acts as an agent of educational change. They find two major consequences stemming from the culture of each school. First, each culture gives expression to a normative goal that aims at shaping the way students understand themselves and from issues of ministry practice. Second, each provides a "cultural tool kit" of knowledge, practices, and skills that students use to construct strategies of action for the various problems and issues that will confront them as pastors or in other forms of ministry. In the concluding chapters, the authors explore the implications of their findings for theories of institutional culture and professional socialization and for interpreting the state of religion in America. They identify some of the practical dilemmas that theological and other professional schools currently face, and reflect on how their findings might contribute to their solution. This accessible, thought-provoking study will not only illuminate the structure and process by which culture educates and forms, but also provide invaluable insights into important dynamics of American religious life. Introducing students to the core philosophical issues surrounding modern physics and the ideas, which have shaped our current understanding of the subject, the book is based on lectures by H. W. Watson and sets out to illuminate and implicate the inextricably entwined nature of philosophy and physics and the importance of logic.

Some years ago, in the course of a general critique of what has sometimes been referred to as the covering law theory of explanation, I made the claim that perfectly satisfactory explanations can often be provided by indicating only one or a few necessary conditions, where we remain ignorant of the sufficient conditions, of what we nevertheless claim to understand. What seemed to me one identifiable type of such explanations I called “explaining how-possibly,” because it was a type more naturally (...) given in response to the question how it could be that a certain thing happened than to the more familiar question why it did so. In the present paper I propose to review certain objections to this thesis which have come to my attention in the interval, and to make such concessions as these seem to require. In fact, the concessions will be minor, since the general thesis still seems to me quite defensible. (shrink)
